The most important developments for Aier Eye Hospital Group during this refresh window should not be reduced to the headline decline in reported profit. Three separate changes matter: patient traffic and core service revenue continued to grow in 1H26 while a sizable tax-related self-review charge materially depressed reported earnings; the company proposed converting 18.56 million previously repurchased shares from incentive use to cancellation; and an AI agent for thyroid eye disease entered initial clinical use, extending Aier's digital ophthalmology capabilities into a real diagnostic workflow.
For 1H26, revenue reached RMB 11.946 billion, up 3.81% year over year. Net profit attributable to shareholders was RMB 1.357 billion, down 33.83%, while adjusted attributable net profit was approximately RMB 1.515 billion, down 25.75%. The interim report and related coverage indicate that approximately RMB 756 million of tax-related expenditure from a self-review was a major drag on reported earnings. At the same time, outpatient visits reached 10.0195 million, up 8.34%, and surgeries reached 888,700, up 1.11%. Refractive surgery revenue was RMB 4.909 billion, up 6.32%, while optometry revenue was about RMB 2.89 billion, up 6.25%. The operating trend therefore needs to be separated from the unusually large tax-related impact on the income statement.
Consolidated operating cash flow remained strong at approximately RMB 2.923 billion, although below RMB 3.402 billion in the prior-year period and still materially above attributable net profit. European subsidiary Clínica Baviera generated roughly RMB 1.370 billion of revenue and RMB 214 million of net profit during the period. Broader international expansion, including the pending Brazil transaction, is not a new event inside this refresh window but remains relevant monitoring context.
On capital returns, the board approved on August 27 a proposal to change the intended use of 18,564,521 shares repurchased in 2023 from equity incentives or an employee stock ownership plan to a reduction in registered capital. The shares represent about 0.20% of current share capital. Because shareholder approval is still required, the correct event status is announced, not completed. If implemented, the cancellation would modestly reduce the share count and mechanically improve per-share metrics.
On clinical technology, Aier launched an AI agent for thyroid eye disease on August 15 and began trial deployment in an initial group of hospitals. The tool can generate diagnostic-support suggestions after physicians complete consultations and examinations. This represents a step from data, model and platform development toward actual clinical workflow integration. The near-term financial contribution is likely immaterial, but broad deployment across Aier's hospital network and partner institutions could improve standardization, specialist access and referral efficiency.
Overall assessment: watch / supportive. Core demand remains resilient, with continued growth in outpatient traffic, refractive services and optometry. The AI deployment and proposed share cancellation are incrementally positive. However, reported profitability was heavily distorted by the tax-related charge, and margin, finance-cost and cash-flow trends still require monitoring. The key question is whether underlying service growth can translate into more stable profit and cash-flow expansion once the unusual tax impact rolls off.
